Please fill in the required information at the top of the page and in the last section on the page (marked "Other"), please enter West Hempstead Food Bank. This will ensure that your donation reaches the Food Bank without delay.

- Please sure to write West Hempstead Food Bank on the memo line to ensure the donation reaches the Food Bank without delay.
